HI PATIENTS!

Please note new protocols for in-person dental appointment at our office:

1. All appointments will be scheduled over the phone, email or text - No walk-ins please.

2. Complete both COVID-19 screening and acknowledgement forms before or at your appointment. Send completed forms back to us prior to your appointment if possible.

3. Stay in your car and call us when you arrive if possible. We will you let you in when we are ready for you.

4. Wear you own mask if possible.

5. We will provide you hand sanitizer and check your temperature.

6. After your appointment, please stay in the dental chair until the waiting room is ready.

7. Depending on patient flow at the time, we may ask you to:

   • Proceed to frontdesk for payment and scheduling OR

   • Exit office first and complete payment and scheduling by phone/email/text.
